Automatic Payment Using Electronic Funds Transfer
Electronic Funds Transfer (EFT) allows you to have your monthly premium automatically deducted from a selected bank account. There is no monthly charge, no cost for postage, your bank will transfer your monthly premium payment from your account on the premium due date.
To sign up, simply print the EFT enrollment form and send it with a check marked “VOID” to the address shown on the form.
There is no charge for enrolling.
You will receive an EFT billing statement to confirm your deduction date and the deduction amount prior to your first EFT payment draw. Please continue to pay your current billing amount until you receive the statement.
